This one-day course is a high-fidelity simulation programme and is delivered in collaboration with the College of Anaesthesiologists in Ireland and takes place in the National Simulation Centre at RCSI. It is open to surgeons working in Irish hospitals.
The aims of the course are: • To facilitate a discussion about the impact of error on the patient and doctor • To discuss the notion of the doctor as the 2nd victim • To demonstrate appropriate methods of disclosure of error • To provide the trainee the opportunity to practice open disclosure using role-play • To present the ASSIST model of responding to a medical error
At the end of the module the trainee should be able to: • Respond effectively to the experience of a simulated adverse event • Demonstrate an awareness of the impact of medical error on patients and healthcare workers • Demonstrate the skills to effectively and compassionately disclose an error or adverse event to a patient • Justify and defend the importance of honest error disclosure • Identify and consider the effects of and adverse event on the second victim • Appreciate the importance of facilitating peer support in the event of an adverse event

Free Access: RCSI Surgical Logbook
This service is available free of charge to Surgical NCHDs registered on our CPD Support Scheme. It is completely voluntary for you to use this, but this logbook has been specifically designed by RCSI to support those who wish to build their surgical careers. It is part of our commitment to support all who work in the provision of surgical care to patients in Ireland.
We are delighted to provide surgical NCHDs with access to the RCSI Logbook.

Summary
It is not an uncommon observation that healthcare professionals can have preconceived notions regarding what human factors involves. Some physicians will have been involved with ‘debriefing’ in the context of a managing a challenging clinical case such as a cardiac arrest, or a major trauma scenario.
People often think of non-technical skills, such as communication, leadership, teamwork, situational awareness. But the principles of human factors are equally applicable to technical skills, including our equipment, our staff, our work environment, and the system of our healthcare delivery.
This two-day certificate provides an introduction to human factors in healthcare. Each day has a separate overall theme, and begins with an overall introductory lecture, followed by a series of practical and interactive workshops developing on the key theme.
On completion, successful participants will be awarded an RCSI-accredited CPD-SS Certificate in Pre-hospital Emergency Medicine.

Scholars on our fully-flexible programme have gained a comprehensive set of knowledge and core competencies within Human Factors in Patient Safety which have led to scholars gaining new Human Factors specific roles, working on new healthcare projects within clinical practice and pro-actively implementing systematic workplace changes to the benefit of patient care.
Scholars from a diverse range of healthcare backgrounds will attend online interactive sessions and workshops one day per month. The rest of the multi-disciplinary programme including didactic teaching, discussion boards, reading, and resources are offered online, which you can access in your own time.

A simple, structured handover system can make surgery safer
New research led by RCSI, published in JAMA Network Open, shows that a straightforward four-step handover system - SIPS (Sick patients; ISBAR; Prioritise; Summarise) - improves patient safety in surgical settings.
A simple, structured handover system can make surgery safer.
New research led by RCSI, published in JAMA Network Open, shows that a straightforward four-step handover system - SIPS (Sick patients; ISBAR; Prioritise; Summarise) - improves patient safety in surgical settings.
The study, which was supported by The MPS Foundation and Bon Secours Hospital, Dublin, was conducted across two tertiary hospitals. Through their evaluation of more than 2,200 patients, the researchers found that introducing SIPS led to:
- Better patient vital signs at 12- and 24-hours post-handover
- Fewer handover-related safety incidents, as reported by staff
- No increase in handover duration, demonstrating the system’s practicality in busy clinical environments
Importantly, the SIPS system was widely adopted and sustained over time, offering strong evidence that standardising handover communication can enhance surgical safety while integrating seamlessly into routine practice.
